For the 2023 reporting year, a good 21,000 training contracts were terminated prematurely. The within-year termination rate — the share of those who drop out in the same calendar year in which they started — stood at 7.9 percent in 2023. In 2020 it was 6.4 percent (Source: AG Statistik zur Ausbildung in der Pflege, final report “Die Ausbildungssituation in der Pflege”, May 2025).

One qualification belongs here for the sake of honesty: the full termination rate across all three training years does not yet exist. The report states explicitly that such rates can probably be presented for the first time with the data for the 2024 reporting year. Anyone quoting you an overall drop-out rate for nursing training has calculated it themselves — usually by subtracting graduates from entrants. That is not admissible: the 35,300 graduates of 2025 began in 2022, not in 2025 (Source: Federal Statistical Office, press release No. 231 of 2 July 2026).

What can be said: leaving in the first year is the most common case, and it has become more common. That is precisely where everything an organisation can influence begins.

What the law allows after a termination

For trainees from a third country, residence hangs on the training contract. Section 16a of the German Residence Act governs the residence permit for in-company training and further training. It is granted where the Federal Employment Agency has consented under Section 39 or a statutory exception applies, and it requires proof of sufficient German language skills — unless the educational institution assesses these itself or a preparatory language course is planned.

Two points from this matter in daily practice:

Secondary employment: the residence permit allows employment independent of the training purpose of up to 20 hours per week. Not more — not even in your own organisation.

After a termination through no fault of the trainee, up to six months are available to find a new training place.

Those six months are the difference between a solvable problem and a departure from the country. They apply only where the termination was not the trainee’s fault. If your organisation closes, gives up its training function or can no longer provide practical instruction, that is a different matter from a contract the trainee terminated themselves. The information here is for orientation and does not replace legal advice; the assessment in each individual case rests with the immigration authority.

When a training relationship does end, the sequence is always the same, and the order decides the outcome. First: record the reason and date of termination in writing, because the immigration authority ties the question of fault to it. Second: inform the immigration authority without delay rather than waiting for the residence permit to expire. Third: involve the nursing school, because the theoretical part already completed is credited towards a follow-on contract. Fourth: begin the search for a new training organisation in parallel, because the deadline runs from the date of termination, not from the day all the paperwork is together.

For workforce planning this also means: a trainee whose contract ended elsewhere through no fault of their own is an excellent candidate for your organisation. They are in the country, speak the language, know the school component and need no visa procedure. In practice this group is overlooked, because hardly anyone goes looking for it.

Practical instruction is not optional, it is a figure

The most common reason a nursing training fails inside an organisation has a statutory section: Section 4 of the German Nursing Professions Training and Examination Ordinance requires that at least 10 percent of each placement period is delivered as planned and structured practical instruction.

What that means becomes clear from the requirements placed on the instructing person. They must have at least one year of professional experience in the relevant placement area, hold a vocational-pedagogical additional qualification — for qualifications acquired before 31 December 2019 there is grandfathering from 200 hours — and complete at least 24 hours of further training per year to maintain the qualification (Source: Nursing Professions Training and Examination Ordinance, Section 4).

Work it out for yourself: for a placement block of 400 hours, 40 hours of planned instruction are due. Not supervision in passing, but planned and structured. Whoever does not have those hours in the rota does not have them.

For international trainees this weighs double. They are learning not only the profession but simultaneously the language in a specialist context, the documentation practice and the hierarchies of a German organisation. For them the 10 percent are the floor, not the target.

Language fails in the placement, not in the classroom

The language certificate required by Section 16a of the Residence Act refers to general language skills. Practice demands something else: handover under time pressure, residents’ dialect, technical terms, phone calls with relatives, legally sound documentation. Someone who keeps up in class may still struggle on an early shift.

That is not an argument against international training but a planning parameter. Three things that can be organised inside the organisation and that no law prescribes:

A fixed contact person in the team who is not the practical instructor. The instructor assesses — for questions one is reluctant to put to the assessing person, a second person is needed.

Job-related language teaching alongside the first placement block, not before it. Vocabulary sticks when it is needed the same day.

A written handover template for the first months. It takes pace out of the situation in which most people drop out linguistically.

Why the language question is less often the real obstacle than assumed, we have covered in more detail elsewhere: The language barrier is the most common objection.

What applies after the final examination

The training ends, and so does the purpose of residence. There is a dedicated provision for this transition: Section 20 of the Residence Act allows a residence permit for job-seeking following a stay in Germany. For training in a health profession it is granted for up to twelve months and can be extended once by a maximum of six months. The precondition is that subsistence is secured (Source: Residence Act, Section 20).

From an employer’s perspective this is good and uncomfortable news at once. Good: nobody has to leave the country immediately if the follow-on contract is not yet in place. Uncomfortable: your newly qualified nurse has twelve to eighteen months to choose an employer — and after three years in your house they are a particularly attractive candidate for every competitor, because recognition, language and induction are already done.

Whoever raises the question of taking someone on only at the end of the third year is negotiating from the weaker position. That conversation belongs in the second training year.

What this means for your organisation

Four consequences.

First: the first year decides. The measurable contract terminations concentrate there. Invest instruction, support and attention at the front, not evenly spread across three years.

Second: the 10 percent belong in the rota, not in the intention. Practical instruction that is not planned does not happen — and cannot be evidenced in an inspection either.

Third: know the six-month deadline before you need it. When a training relationship ends through no fault of the trainee, a clock starts running that day. Whoever begins looking into it only then loses weeks of it.

Fourth: settle the question of taking them on early. After the examination, up to eighteen months of search time are open. Your negotiating position is better in the second year than in examination week.

We deliberately give no figures from our own completed training relationships in this article. What we describe is the legal position and the available statistics, not a result we cannot yet show.

Further reading

Nursing trainees from abroad: what your organisation needs

What a nursing trainee costs — and what they bring back

Two paths into nursing, only one needs recognition

Hiring a care assistant: what the market offers

Sources

AG Statistik zur Ausbildung in der Pflege: Die Ausbildungssituation in der Pflege.
